1.17     Immunization Requirements

Immunizations are required for the following diseases for all UT Southwestern current and visiting students with patient contact. See below for the required documentation:

Tetanus/Diphtheria/Pertussis(Tdap)
Measles (Rubeola or Red Measles)
Mumps
Rubella (German Measles)
Hepatitis B
Varicella (Chicken Pox)
Tuberculin skin test (PPD)

One of the following is required for each immunization:

Tetanus/Diphtheria/Pertussis:
Documentation tetanus-diphtheria, acellular pertussis (Tdap) within the past ten years.

Measles (Rubeola or Red Measles):
If you were born on or after January 1, 1957:
(1) Documentation of two doses of measles vaccine administered since January 1, 1968 OR
(2) Documentation of two doses of MMR vaccine administered since January 1, 1968 OR
(3) Documentation of one dose of measles vaccine and one dose of MMR vaccine administered since January 1, 1968 OR
(4) A positive titer (blood test) confirming immunity or evidence of prior infection (include copy of laboratory report)

Mumps:
If you were born on or after January 1, 1957:
(1) Documentation of one dose of mumps vaccine OR
(2) Documentation of one dose of MMR vaccine OR
(3) A positive titer (blood test) confirming immunity or evidence of prior infection
(include copy of laboratory report)

Rubella (German Measles):
If you were born on or after January 1, 1957:
(1) Documentation of one dose of rubella vaccine OR
(2) Documentation of one dose of MMR vaccine OR
(3) A positive titer (blood test) confirming immunity or evidence of prior infection (include copy of laboratory report)

Hepatitis B:
Hepatitis B is required for all students who will have contact with human or animal body fluids. This includes the following programs: 1) Medical, 2) Medical Laboratory Sciences, 3) Blood Bank Technology, 3) Physical Therapy, 4) Physician Assistant, 5) Prosthetics and Orthotics programs.
(1) Documentation of three doses of Hepatitis B vaccine OR
(2) A positive titer (blood test) confirming immunity or evidence of prior infection (include copy of laboratory report)

Varicella (Chicken Pox):
(1) Documentation of two doses of varicella vaccine OR
(2) A positive titer (blood test) confirming immunity or evidence of prior infection (include copy of laboratory report) OR
(3) History of disease validated by yourself, your parent/guardian, or health care provider. You may enclose a letter written by you, your parent/guardian, or healthcare provider stating the approximate date or year that you had the disease.

Tuberculin skin test (ppd):
(1) If you have never had a positive ppd: Documentation of one purified protein derivative (ppd) tuberculin skin test within the past year (must include the millimeters of induration whether positive or negative). If the skin test was positive, documentation of a chest x-ray is required. OR
(2) If you have a history of a positive skin test: Include the approximate month, day, and year of positive test on the Immunization History form, and documentation of a chest x-ray taken after the positive skin test.

1.18     Inclement Weather

The medical center campus will not close because of inclement weather. Students will need to make their own decisions regarding hazardous traveling conditions; however, student responsibilities are not obviated by weather conditions. If a student believes traveling in such weather is hazardous, the student will be expected to make up missed class work.

1.19     Intellectual Property

The Intellectual Property Policy, and related policies, apply to students as well as to faculty and staff. It provides, in part, that all inventions and discoveries, including computer software, developed by students are owned by UT Southwestern. The Office of the Vice President for Legal Affairs and Technology Transfer handles all legal and business matters involving protection and commercialization of the invention or discovery. After costs are deducted, income generated from intellectual property is split fifty-fifty between UT Southwestern and the student who is the inventor.

The complete policy, as well as related policies, can be found in chapter 2 of the Handbook of Operating Procedures. The UT System policy, to which UT Southwestern adheres, is located in the Regents' Rules, Part Two, Chapter XII.

1.20     Leave of Absence

The Associate Deans for Student Affairs may grant a leave of absence for up to one year upon request. (Of course, there is a form to fill out.) Students with academic deficiencies wishing to take a leave of absence require the permission of the student promotions committee; these requests may be brought before the student promotions committee through the Associate Deans for Student Affairs. Leaves in excess of one year require the approval of the student promotions committee. Failure to return at the predetermined end of a leave of absence is considered withdrawal, thus requiring re-application to return. While on leave, the individual is not considered to have student status.

Prior to leaving, students receiving financial aid must see his/her financial aid counselor to understand and insure compliance with loan deferment and repayment requirements. Students must also turn in all keys, pagers and other checked out materials.
