Immunization Requirement Tetanus/Diphtheria One dose within the last ten (10) years. Measles (Rubeola or Red Measels) If you were born on or after January 1, 1957, you must have either documentation of two (2) doses of measles or two (2) doses of MMR vaccine (separated by at least 30 days) after 12 months of age, a positive titer (blood test) or history of disease. Mumps If you were born on or after January 1, 1957, you must have either documentation of one dose of mumps or MMR vaccine after 12 months of age, a positive titer (blood test) or history of disease. Rubella (German Measles) You must have either documentation of one dose of rubella or MMR vaccine after 12 months of age or a positive titer (blood test) result. Hepatitis B A series of three (3) injections or documentation of a positive titer. Hepatitis B is required for the following students only: 1) Medical students, 2) Medical Laboratory Sciences students, 3) Physical Therapy students, 4) Physician Assistant Studies students, 5) Prosthetics and Orthotics students. Note: There must be four (4) weeks between the 1st and 2nd doses and at least five (5) months between the 2nd and 3rd doses. Varicella (Chicken Pox) You must have two (2) immunizations, a report of a positive titer (blood test) or a report of illnes. Tuberculin (tbe) skin test (PPD) You must have a skin test (5 TU Mantoux) within the past year or if you have a history of a positive skin test, you must provide documentation of an x-ray taken after the positive skin test.
- Please note that your immunization record must include month, day and year on all vaccinations.
- History of disease for rubeola or mumps must be accompanied by a letter from a physician licensed to practice in the United States.
- Provide a copy of the laboratory report on all immune titers.
